What It Includes:

- All x-rays
- Two cleanings/year
– Adult/Child Prophylaxis
– Periodontal Maintenance - Two fluoride applications
- 20% discount on all procedures!
*Periodontal maintenance recommended every 3-4 months. 3rd and/or 4th periodontal maintenance will be at 20% discount*
Program Exclusions and Limitations:
The discount plan is a membership plan, not a dental insurance plan.
It cannot be used:

- In conjuction with another dental plan or insurance plan
- For services or injuries covered under workman’s compensation
- For referrals or specialists
- At another dental office

Annual Fee/Duration

- $300 individual
- $500 family (up to four people)
– Household with children aged 20 or less - Plan starts the day of agreement and full plan payment
- Valid for one year ( e.g. start date of June 3, 2026 has end date of June 3, 2027)
